Support Coordination & Specialist Support Coordination
PSG provides Support Coordination and Specialist Support Coordination services across Perth Metro and wider Western Australia. Support Coordination is funded within the Capacity Building stream of your NDIS Plan. In addition to our services across Western Australia, PSG also works with a number of participants interstate.
The purpose of Support Coordination is to help you and your family access the supports available to you so you can achieve your goals. Navigating the NDIS can be complex; we support you by reducing the pressure so you can focus on what matters to you, because every moment matters.
Our service takes a practical approach to identifying your needs, wants, and goals. We focus on building strong, collaborative relationships with you and the people involved in your support network. At PSG, we believe that working together with participants, families, and service providers is key to creating sustainable support structures and achieving positive outcomes.
At times, your needs may require a more specialised approach, and you may be able to access Specialist Support Coordination. PSG Specialist Support Coordinators are experienced in supporting complex situations, including housing, behavioural support, mental health, justice involvement, Child Protection and Family Services, Early Childhood Intervention, support worker services, health and hospital systems, aged care, employment, and other significant life challenges.
PSG Support Coordinators focus on clear communication and practical, hands-on support. We aim to use your Support Coordination funding effectively by using technology to offer flexible options, including fully remote services via Microsoft Teams, Zoom, email, and phone. This reduces unnecessary travel, increases flexibility for participants and families, and supports better use of your plan. For new referrals, we offer an initial consultation to understand your needs and determine the right approach.

Roles of our support coordinator
- Help you understand and implement your NDIS plan
- Connect you with NDIS-funded and mainstream services
- Build your capacity and confidence to engage with providers and the NDIS
- Work collaboratively with stakeholders to support you to achieve your goals
- Support you to exercise choice and control in your supports
- Prepare reports and supporting documentation for the NDIS when required
- Assist with plan reviews and preparation for upcoming reviews
- Support you to request a plan review or address concerns with your current funding
- Monitor your supports to ensure they are working effectively
- Help you build independence and capacity over time
- Provide clear, practical guidance so you can make informed decisions

Regional Services
PSG provides Support Coordination and Specialist Support Coordination to participants living in regional and remote areas of Western Australia. This includes locations across the South West, such as Bunbury, Busselton, Margaret River, Albany, Bridgetown, Manjimup, Boyup Brook and surrounding areas, as well as northern communities including Broome and Kununurra.
We combine flexible remote delivery with scheduled in-person visits to ensure consistent, high-quality support that suits your local context. Our coordinators bring local knowledge and experience, offering both remote and face-to-face support depending on your location. We regularly travel to key regional areas to provide in-person services and maintain strong community connections.
