Support Coordination & Specialist Support Coordination
PSG provides Support Coordination and Specialist Support Coordination services across the Perth Metro area. Support Coordination is funded within the Capacity Building stream of your NDIS Plan.
The purpose of Support Coordination is to help you and your family access all the supports that are available to you so that you can achieve your goals. Navigating the NDIS can be challenging; we support you through this by taking some of the pressure away so you can focus on living a life of purpose, adventure, and reaching your goals.
Our service provides a practical approach to identifying your needs, wants, and wishes. We work to build strong, collaborative relationships with the people and services in your support network. At PSG, we believe that fostering meaningful connections with participants, families, and providers while working together as a team is key to creating sustainable support structures and maintaining positive outcomes.
At times, your needs may require a more specialised approach, and you may be able to access Specialist Support Coordination. PSG Specialist Support Coordinators are highly skilled in managing and supporting complex situations, including housing issues, behavioural issues, mental health concerns, justice involvement, Child Protection and Family Services involvement, Early Childhood Intervention, Support Worker Services, Health/Hospital, Aged Care, Employment, and other significant life challenges.
PSG Support Coordinators are equipped with the latest technology to ensure seamless communication and efficient support delivery. PSG wants to ensure that your Support Coordination funds are used in the most economical way; therefore, we offer flexible options such as a fully remote service where we use Microsoft Teams/Zoom, emails, and phone calls to deliver your service. By embracing digital tools and avoiding unnecessary printing, we strive to reduce our environmental impact. This approach also cuts down on travel costs and provides participants and families with greater flexibility in meeting times.

Roles of our support coordinator
- Help you understand your NDIS plan.
- Connect you with NDIS and mainstream providers.
- Build your capacity and confidence in speaking with the NDIS and services.
- Liaise with stakeholders to ensure a collaborative approach to your services.
- Support you in exercising choice and control in your life.
- Provide relevant reports or support letters to NDIS as required.
- Provide support for your plan review.
- Assist you with requesting a plan review or addressing concerns about your current NDIS funding.

Regional Services
PSG provides Support Coordination and Specialist Support Coordination to participants living in regional and remote areas of Western Australia. This includes locations across the South West — such as Bunbury, Busselton, Margaret River, Albany, Bridgetown, Manjimup, Boyup Brook and surrounding areas — as well as northern communities including Broome, Derby, and Kununurra.
We combine flexible remote delivery with scheduled in-person visits to ensure participants receive consistent, high-quality support that fits their local context. Our coordinators bring local knowledge and experience, offering both remote and face-to-face support depending on your location. We regularly travel to key regional areas to provide in-person services and maintain strong community connections. Participants can also access support with NDIS access, plan implementation, and review preparation.
