Coordination

& Specialist Support Coordination

Support Coordination is funded within the Capacity Building stream of your NDIS Plan. The purpose of Support Coordination is to help you and your family access all the supports that are available to you so that you can achieve your life goals.   

Navigating the NDIS can be challenging; we support you through this by taking some of the pressure away so you can focus on living a life of purpose, adventure and reaching your goals. Our service provides you with a practical approach to achieving your goals. We do this by helping you identify your needs, wants and wishes and helping you identify whom you want in your support structure and support to maintain these relationships.

At times, your needs may require a more specialised approach, and you may be able to access Specialist Support Coordination. For example, PSG Specialist Support Coordinators are highly skilled in managing and supporting complex situations such as housing issues, behavioural issues, mental health concerns, forensic involvement, Child Protection and Family Services involvement and other significant issues that may occur throughout life, which requires a more experienced approach.

Propel Support Group wants to ensure that your Support Coordination funds are used in the most economical way, therefore we offer flexible options such as a fully remote service where we use Microsoft Teams/Zoom, emails and phone calls to deliver your service. This is a popular option for a lot of people as it not only cuts down on travel costs but also gives clients & families more flexibility in meeting times as our remote Support Coordinators can work outside of typical business hours.
